Introduction
 The following questions are intended to help us determine, which surveys and discussions your experiences and expertise would be best suited to. This also enables us to reduce the number of phone calls and e-mail messages you may receive as a result of your participation in the Member Advisory Panel.

Participation may involve e-mail surveys that will vary between 20 and 60 questions per survey, or phone-based focus groups and in-depth interviews that will vary between 15 and 30 minutes. You can specify your preference for web-based surveys or phone studies at the end of this form.
The EAA Research and Analysis Department follows the highest privacy standards and ethical guidelines. To learn more, refer to the end of this form.

In an effort to maintain your privacy, this contact information will be kept in a separate and secure database. Although some of this information may be available on the EAA Membership database, the EAA Research and Analysis Department would rather maintain your anonymity by using you as the sole information source.
We would also like to know the ideal times to contact you so as not to make participation an inconvenience for you.
